What is ?
This is a Windows-based charting system designed for single physician use that allows the provider to “write”
in a patient chart, almost exactly like a paper chart. This means that all the physician’s records are instantly
available and completely portable, allowing access anytime, day or night! Because templates are used, handwriting is minimized,
and legible, well organized records are quickly created.
How does it work?
uses a Microsoft Windows Tablet
computer which the user writes on directly with a stylus. This “writing” applies digital ink over a patient care
template. The templates are based upon the Doc-U-Chart paper templates. Once completed, a patient encounter can be saved, printed,
and reviewed at will.
What do I need?
A Windows TabletPC® which runs a special version of Windows XP is required. We recommend the fastest processor you can find, and a
60 GB hard drive. Your Tablet should have integrated wireless capability. You will need a Printer/Scanner to print your chart pages if
desired, and to take advantage of our scanning capability. A WindowsCE PDA, such as a Dell Axim, is necessary to allow a nurse or assistant
send patient data to the device.
How do I use it?
The program works very much like a paper chart. A patient is selected, and the choice of a template is made based upon the patient’s
presentation. The chart pages can be scrolled forward or backward, or a particular encounter can be quickly located and viewed.
How is the PDA used?
The PDA is used for entering vital signs and selecting the encounter template. The nurse can access the Tablet database from a WindowsCE device
over a wireless connection. She records patient data on the PDA, and sends it, along with her template selection, to the Tablet. This all can
happen while the physician is using the Tablet for another patient encounter.
Does it do more than Encounters?
Yes! The program can be used to store all parts of your patient’s record. A scanning utility is built into the application which allows any
paper form to be scanned into the device, and catalogued. Images are saved to a particular category, such as Lab, Imaging and X-ray, Consultations,
Hospital Records, and so on. Each category allows the selection of a ‘Type’. For example, the Imaging and X-ray Category has the Cat Scan,
MRI, Plain Films, and Ultrasound sub-types, to name a few. This allows the entire category for a patient to be viewed, or only the portions of the
Category that are pertinent.
Is it secure and HIPAA Compliant?
Absolutely. Your Tablet can be programmed to be password protected. In addition, our application is password protected. Lastly, our SQL database is
password protected and immune to prying or copying. As needed, we can work with your network administrator to facilitate archiving and backups.
Where is the data stored?
The chart is stored on the device. A 60 GB hard drive will easily hold all the chart data for a single physician’s office practice. We strongly
recommend daily full backup of your tablet to an external storage device such as a Maxtor External Hard Drive. We can work with your network administrator
to archive chart images to other servers, where your chart can be available to others in your organization.
Can I import my data?
Yes. If you have patient demographics in a database, we can work with your database administrator to copy your patient demographics to your Doc-U-Chart
data base. If you already have a database of digital images that you would like to transfer to your Tablet, we can help with this.
How many Templates are there?
We have over 80 exam templates for a wide variety of presentations. This includes an extensive set of pediatric templates that are age range specific
(Infant and Toddler, Child and Adolescent). There are also Problem List templates, and age and sex specific PFSH forms (Past, Family and Social History).
Also included are Immunization Record forms. Most of the forms in the Doc-U-Chart Paper Template set have a counterpart on the TabletPC®.
